| Enrollment: |
Team is an annual comittment! Continuous enrollment is required. Please notify coach and office of changes
in advance. |

To remain on team the minimum enrollment is the
monthly team fee (no tuition coupons required). |
|
|
| Billing: |
Monthly team fees are automatically billed to
each athlete's account, continuously until notification of discontinuation. |
|
|
It is the client's responsibility to notify the
coach and office (registrar) of changes in the number of practice days |
|
|
per week prior to the start of each month. |
|
|
|
|
|
| Total Cost: |
Annual membership + monthly team fees + daily
tuition coupons + coaches travel expenses + meet expenses + uniform |
